The word "shemale," on the other hand, is viewed almost exclusively as a term specific to the adult film industry. It typically describes a pre-operative trans woman—someone who has developed female secondary sexual characteristics (often through hormone therapy or surgery) but retains male genitalia. While functional in the world of pornography, the term is frequently considered offensive and derogatory within the general transgender community, as it reduces a complex human identity to a sexual function.

Following Stonewall, Rivera and Johnson founded Street Transvestite Action Revolutionaries (STAR) in 1970. STAR provided housing, food, and community to homeless queer youth and trans women in New York.
